Maternal deaths linked to poor NHS care, says report

An independent inquiry has found that the deaths of 17 women in London Hospitals over an 18 month period could have been avoided if they had received better care.

The BBC's Panorama programme was given exclusive access to the report on substandard maternity care, which was commissioned by NHS London.
