NHS apology to murder victim family
An NHS mental health trust has apologised to the family of a man murdered by one of its patients.
Brian Maddock stabbed to death his former partner Michael Naylor at their home in May 2010.
A review into the case found that the Manchester Mental Health NHS Trust failed to warn Mr Naylor his life was at risk, and failed properly to assess the risk Mr Maddock posed to him.
