End-of-life care 'not good enough'

Doctors have found "deeply worrying" standards of NHS care are being provided to the dying.

The care given to people dying in hospital is "deeply concerning", according to doctors who have carried out a review of standards in England.

The audit by the Royal College of Physicians looked at more than 6,000 deaths.
