Southern Health: Warning issued to criticised NHS trust
Southern Health NHS Foundation Trust has been issued a warning by the Care Quality Commission and told to "significantly improve", after an inspection found that the trust's investigation of incidents, including deaths, was not robust enough.
The report is the latest in a series of investigations sparked by the death of Connor Sparrowhawk, who died in the trust's care in Oxford in 2013.
Southern Health said it was "completely focused" on tackling the concerns.
