Southern Health 'failed to protect patients' says report
A health regulator has strongly criticised a major NHS trust in England which provides mental health care.
The Care Quality Commission says the leaders of the Southern Health Trust failed to protect patients after the death of a 15-year-old boy following an epileptic seizure.
The father of one woman who died told the BBC's Michael Buchanan that the system let her down.
