Deaths from domestic abuse to be automatically reviewed

Every death from domestic abuse in England and Wales is to be automatically reviewed.

This is in an effort to make sure lessons are learned about preventing violence in the home.

The Director of Public Prosecutions has called for more action to prevent abuse, warning that teenage women are most at risk.
