Agencies admit 'serious mistakes' over murdered toddler
Fourteen different organisations missed opportunities to save a three-year-old boy killed by a couple who were meant to be caring for him.
Ryan Lovell-Hancox received more than 70 injuries at the hands of the couple who had been paid to look after him by his mother.
He died over two years ago from a brain injury.
Christopher Taylor and Kayley Boleyn were jailed for murder and child cruelty.
