Alison Hume's death in Galston mineshaft 'preventable'

A woman trapped in a mineshaft in Ayrshire might have survived if it had not been for a series of failings by Strathclyde Fire Service, an inquiry into her death has concluded.

Alison Hume fell down a disused mineshaft in Galston in 2008, but health and safety rules delayed a rescue for six hours.

Mrs Hume died after suffering a heart attack while being brought to the surface.
