Mother and toddler deaths 'could have been prevented'
The partner of a pregnant woman, who was fatally stabbed alongside her 23-month-old son, says their deaths could have been prevented.
Robert Barlow believes different agencies should have done more to make Rachael Slack aware of the risks posed by her former partner.
However a Serious Case Review being published on Thursday says their deaths could not have been prevented or predicted.
