'They gave her none or the wrong medication'

Some of the most vulnerable patients, including frail and elderly people, are enduring harrowing ordeals when they leave hospital, according to a report by the ombudsman.

The independent arbitrator found that NHS patients in England are being sent home from hospital afraid and with little support.

Angela Little's 80-year-old mother was one of those affected; Angela describes the care that was available.
