Alison Hume's death in mineshaft 'avoidable'
A woman trapped at the bottom of a mineshaft might have survived had it not been for a series of failings by Strathclyde Fire and Rescue Service, an inquiry has found.
Alison Hume fell down the shaft and remained there for six hours because the team sent to rescue her believed their own rules stopped them from using their safety equipment.
Fire service commanders declined to apologise to the victim's family, who have demanded a public inquiry.
