Faulty gearbox caused Super Puma crash

The official report into a North Sea helicopter crash which killed 16 people has been released.

The report said a "catastrophic" gearbox failure was the main cause of the accident.

The findings also noted that operators had considered replacing the gearbox but did not.

The Super Puma helicopter had been flying back to Aberdeen when it came down off Peterhead in April 2009.
