Lossiemouth crash air crewman had 'fear of flying'
Air accident investigators have recommended that the UK defence secretary orders a review of safety issues within the MoD.
The Military Aviation Authority has released its official report into a mid-air collision, between two Tornado aircraft from RAF Lossiemouth in 2012, in which three air crew died.
One of the four crew involved was suffering from a fear of flying at the time of the accident.
Investigators have said they do not believe it was the cause of the accident over the Moray Firth but is considered one of 17 contributory factors which made the accident more likely to happen.
