Hit-and-run victim Karina Menzies's family 'let down'

The sister of a woman killed by a hit-and-run driver with paranoid schizophrenia has said both he and the victim's family were "let down" by mental health services.

Karina Menzies, 31, died when Matthew Tvrdon went on a "journey of mayhem" in Cardiff on 19 October 2012, injuring others at five locations.

The 32-year-old was detained indefinitely by a judge last year under the Mental Health Act after he admitted manslaughter on the grounds of diminished responsibility.

A review of the case by Healthcare Inspectorate Wales has said there were a number of shortcomings in Tvrdon's care, but said the death of Ms Menzies could not have been predicted.

Her sister Samantha told BBC Wales she disagreed, and believed more could have been done to prevent the tragedy.
