Drug errors cause appalling harm and deaths, says Hunt

Researchers from Manchester, Sheffield and York universities have collated a study in which data suggests that millions of drugs errors may be being made.

Health Secretary Jeremy Hunt says the errors "cause appalling levels of harm and deaths".

According to the study, GPs, pharmacists, hospitals and care homes may be making 237 million errors a year - the equivalent of one mistake made for every five drugs handed out.

The research revealed that most caused no problems, but in more than a quarter of cases the mistakes could have caused harm.

Woman in hospital after collision in Lancaster

A woman is in a "serious but stable" condition after a collision in Lancaster yesterday.

Emergency services were called to Main Road at about 18:05 GMT after reports of a collision involving a Toyota Yaris and a pedestrian.

The lady, in her 80s, suffered hip injuries and was taken to Royal Lancaster Hospital where she is currently receiving treatment.

Police are asking for anyone with information to contact them.

Liverpool prison staff to be given more training after inmate's death

Staff at HMP Liverpool are being given more training in spotting mental health issues and preventing self-harm by inmates, after a third prisoner took his own life in the last six months.

The governor has said immediate action is being taken to deal with the challenges at the jail after inspectors recently described squalid conditions there.

The latest death at the prison was that of Tony Paine, who was found hanged in his cell on Monday.

He had previously been a mental health patient.

The family of seriously ill toddler Alfie Evans are preparing to mount an appeal after losing a High Court battle over his life-support treatment.

Doctors were due to withdraw treatment today but lawyers for his family will take the case to the Court of Appeal next Thursday.

Alfie has been in a coma for 14 months with an undiagnosed brain condition, at Alder Hey Children's Hospital in Liverpool.

The family thanked people on the Alfie's Army website who had raised £10,000 to fund an appeal.

HMP Liverpool: Mum says son was 'treated like an animal'

The mother of a prisoner who killed himself at HMP Liverpool has said her son was "treated like an animal".

Tony Paine, 35, died in hospital after being found in his cell on Monday.

He is the third inmate to have died by suicide at the jail since inspectors made an unannounced visit in September, finding "squalid" living conditions and major management failures.

Lancashire Care NHS Foundation Trust said an independent investigation into the death will take place.

Legal action against Blackpool fans has been halted by the club's owner Owen Oyston.

Mr Oyston, who is the focus of a continuing campaign by fans wanting him to quit the club, met with an umbrella group representing supporters he had sued.

Justice4Fans thanked Mr Oyston for "recognising the importance" of bringing the claims to a conclusion.

A club spokesman confirmed Mr Oyston had decided to end the litigation.

The virus has been linked to severe birth defects and was declared as a global medical emergency by the World Health Organization in 2016.

The Universities of Liverpool and Manchester and Public Health England will work on the £4.7m project to find a "ready-to-use vaccine".

Project leader Prof Neil French said it "would dramatically reduce the threat that we face from Zika".

At the time, Norma was relatively new to the sport and had "yet to try it in the big outdoors".

Two police cars have smashed into each other while following a suspected stolen vehicle in an off-road chase.

The Cheshire Police cars collided off the M56 Princess Parkway in Wythenshawe, Manchester, shortly before 14:00 GMT on Wednesday.

One officer suffered minor injuries and was taken to hospital as a precautionary measure, the force said.

Three males and two females aged between 16 and 24 were arrested on suspicion of motor vehicle theft.

'Key member' of voodoo trafficking gang arrested in Manchester

A woman suspected of helping run a sex trafficking gang which made victims eat raw chicken hearts in voodoo rituals has been arrested in Manchester.

The National Crime Agency said the Spanish national was a "key member" of an international sex ring.

Police in Spain arrested 11 people as part of the network that allegedly trafficked women from Nigeria.

The woman arrested in Manchester was held under a European Arrest Warrant on 2 February.

Her 78-year-old partner suffered cuts to his head when they were hit by a silver Ford Fiesta as they crossed between Union Road and Lord Street at about 16:05 on 20 February.
