Dudley Partnerships for Health LLP put in special measures
A GP practice has been put into special measures after inspectors found it to be inadequate.
Dudley Partnerships for Health LLP did not provide a safe and well-led service and "must improve how responsive, caring and effective it is".
The Care Quality Commission (CQC) said inspectors found "significant areas of concern" at the Quarry Road surgery and will monitor the practice.
The practice acknowledged the action required, the CQC said.
"We will continue to monitor this practice and we will inspect again in six months to check whether improvements have been made," a spokesman said.
"I am hopeful that the practice will do what is required for the sake of its patients, but if we find that the service remains inadequate, we will consider taking further action."
