Home help for hospital patients to cut readmissions

Patients leaving hospital are to be given a week of support to settle at home, under a new contract agreed by the NHS and Surrey social services.

The British Red Cross and the social enterprise charity Home Group are to provide seven days of home help.

Surrey County Council said the extra support would reduce the need for hospital readmissions.

The one-year, £335,000 agreement covers all Surrey's acute and community hospitals and starts in October.

Adult social care councillor Mel Few said: "Going home after a stay in hospital can be a daunting prospect, which is why it was essential to ensure support is on hand seven days a week."

He said "everyone pulling together as one team" would ensure taxpayers' money went further when "budgets are under growing strain from rising demand".
