Man died at Glan Clwyd Hospital after 'lapses' in care
A 43-year-old man died in hospital following "repeated lapses in his care and treatment", an inquest has heard.
John Yee, from Prestatyn, Denbighshire, died at Glan Clwyd Hospital in 2013.
The Llandudno inquest was told Mr Yee went days without food, had delays in medication and was "inappropriately" transferred to a general medical ward.
The assistant coroner said he died from aspiration pneumonia, brought on by an underlying medical condition, and recorded a narrative conclusion.
The inquest heard a serious incident review carried out by Betsi Cadwaladr University Health Board found the failings following his admission on 24 February 2013.
Mr Yee, who suffered from a medical condition requiring 24-hour care, later died on 2 March that year.
However the assistant coroner for north west Wales, Nicola Jones, said she would not be writing to the health board as it had taken "a robust approach" to putting preventative changes in place.
