Bristol beat Rotherham at Abbeydale to secure a 56-36 aggregate win and reach the Championship play-off final.
Having won the first leg 32-20, Bristol trailed 9-6 at half-time in the second leg after three Tom Barrett penalties.
Jamie Broadley then scored an interception try to extend Rotherham's advantage on the day, and reduce their aggregate deficit to only two points.
But tries by Marco Mama and Jack Tovey restored Bristol's grip on the tie and secured a final date with Worcester.
Andy Robinson's side have opted to face the Warriors in the first leg at Ashton Gate on Wednesday, 20 May, with the second leg seven days later at Sixways.
Bristol are looking to return to the Premiership for the first time since they were relegated in 2009, when they finished bottom with only two wins from 22 matches.
This is their fourth appearance in the Championship play-offs, having previously been beaten by Exeter Chiefs and London Welsh in the final, and Cornish Pirates in the semi-finals in 2012.
Barrett and Gavin Henson traded penalties in a tense first half, before Broadley's interception try kept Titans' hopes of making the final intact.
But scrum-half Dwayne Peel sprinted down the blindside of a scrum to set Mama clear for Bristol's first touchdown, and replacement Tovey ran through to make the tie safe.
