Failures in prisons 'at all levels'

The chief inspector of prisons has said there has been a failure of management at all levels in the prison service. Peter Clarke told the Today programme failures were at a "local, regional and national level".

A review of Liverpool prison says conditions are the worst inspectors have seen.
