Campaigners are calling for a public inquiry into why more health authorities are not being prosecuted when patients die in their care.

A family whose daughter died as a result of failings by a Scottish health board have joined calls for a public inquiry into why more health authorities are not being prosecuted when deaths occur.

Janette and Ian Black are supporting a campaign by a former health regulator who says dozens of opportunities for prosecutions are being missed.
